What is the biggest problem in orthopedic surgery? According to Tian Wei, dean of Beijing Jishuitan Hospital, “how to see deep structures, achieve three-dimensional positioning, perform precise operations on patients, and not harm nerve tissue” is the biggest “difficult point” faced by orthopedic surgeons.

Only by the human eye, this difficulty can not be overcome, Tian Wei found a "robot doctor" to help. Tian Wei established spine surgery in Jishuitan Hospital in 1997. For more than 30 years, he has been committed to the precision, minimally invasive, intelligent research and application of orthopedics diagnosis and treatment. He is the founder and pioneer of orthopedic surgery navigation and robotic clinical field in China.

On December 18, 2016, at the 18th Osteopathic Conference of the Chinese Medical Association and the 11th COA International Academic Conference, Tian Wei presented the world's third-generation domestic orthopedic surgery robot called “Scorpio”. This is the first universal orthopedic navigation surgery robot that can be used in many parts of the spine, pelvis, acetabulum and limbs. Because of Tian Wei's outstanding contributions in the field of orthopedic surgery navigation and robotics, he won the National Innovation Award Medal this year.

Direct orthopedic surgery

With the reform and opening up, China's surgical science has made rapid progress, and a large number of doctors have "goed out" to learn advanced foreign technology. Tian Wei is one of them. After returning from studying in Japan in 1995, he returned to Jishuitan Hospital.

Recalling the scene of the year, Tian Wei told the reporter of the Chinese Journal of Science and Technology: "At that time, there were more conservative treatments. The progress of surgery was reflected in two aspects. One was the introduction of surgical tools and special tools for orthopedics, such as magnifying glasses and electric knives. China does not; the other is to learn a lot of internal fixation techniques, such as steel plate fixation, etc., the advancement of this large series of technologies has shortened the treatment time for patients."

Orthopaedics is a deep operation with complex structures. Different doctors have tried different methods in order to improve the accuracy of surgery. Tian Wei said: "I remember that when the robot 'Deep Blue' defeated humans and won the chess game, it left a deep impression on me, which made me realize that intelligent technology has the potential to overcome human defects."

Since then, Tian Wei has been studying navigation technology and robotics. In 2002, Jishuitan Hospital took the lead in introducing computer navigation assisted surgery technology in China. Tian Wei team broke through the basic key technologies such as bone trauma robot configuration, dual plane positioning algorithm and robot clinical operation specification, and successfully developed the first orthopaedic robot in China. Prototype system - small biplane orthopedic robotic system.

On December 9, 2004, China's first robot-assisted orthopedic surgery, intramedullary nail fixation for humeral fractures, was successfully completed in Jishuitan Hospital.

Industry-university research and medicine "multi-party" cooperation

A first-line clinical doctor developed a robot, which in many people's eyes is "a fantasy", but Tian Wei has completed this seemingly impossible task.

Talking about the secret of combining production, study and research, Tian Wei told reporters: "This is not done by a doctor alone. We are a team. Beijing Jishuitan Hospital, Beijing University of Aeronautics and Astronautics, and Chinese Academy of Sciences are all involved. And, we combine each other. It must be done from the very beginning of the cooperation, not the individual, and finally integrated."

In this way, the clinical experience of the clinician is the dominant, the research unit turns the doctor's idea into reality, and the research results are brought to market through the enterprise.

Through various efforts, Tianzhihang Company was established in 2005. This is a company established for the transformation of orthopedic surgery robots. It also marks a key step for Tianwei team to transform the laboratory and clinical research results into the market.

The development of orthopedic robots involves many disciplines such as medicine, mechanics, and computational science. Contradictions and conflicts in the process of cooperation are also inevitable. Tian Wei told reporters: "Everyone's professional field is different. At first, there are often poor communication situations, but only by overcoming difficulties and persisting, the innovation results can be continuously promoted and transformed into productivity."

On August 12, 2015, the world's first robotic assisted internal fixation for complex upper cervical deformity was performed at Beijing Jishuitan Hospital. The error was less than 1 mm. The surgeon was Tian Wei.

Under the cooperation of the team, the first orthopaedic robot product standard in China, "Orthopaedic Robot Navigation and Positioning System" was born, and obtained the first medical robot III (highest grade) device registration certificate in China, "Intelligent orthopedics based on image navigation and robot technology" The establishment and clinical application of the surgical system also won the second prize of the National Science and Technology Progress Award.

Intelligent technology will profoundly affect human life

The successful development of orthopedic robots has enabled more and more doctors to start using this technology for treatment. How to treat the relationship between people and robots is also one of Tian Wei's concerns.

He said: "I think that the machine was invented by humans. At this stage, the machine is a human assistant. It is impossible for the robot to surpass human beings as a whole. But humans can never reach the point where smart technology can make up for it, and it will profoundly affect humans. Every area of life."

In Tian Wei's eyes, intelligent technology is the trend of the times, with a subversive and revolutionary significance. Science and technology workers in all fields in China must actively participate in it.

At the same time, as a science worker, he believes that letting the public believe in science and let the people understand the problem is also an indispensable part of popularizing intelligent technology. Tian Wei said: "It is very difficult to do science. To let the people understand professional issues, they need to continue to work. In addition to science and technology workers, the media and social organizations must act."
